What this website collects

Clinical information
Any clinical information you share during an assessment, including your history, your medication, the operation and its date, and the state of the wound, forms part of your medical record at Chongqing West District Hospital and is held under the hospital’s privacy and records policy. It is used for your care and for the review of your own treatment, and for nothing else without your consent.
Photographs and wound records
Photographs and wound records are taken at intervals because a change in the edges, the discharge or the surrounding skin is judged by comparison rather than from memory. They are not published, used for teaching or used in promotional material without a separate, specific consent that you may refuse without affecting your care.
Your choices
You may ask what information is held about you, ask for a correction, or withdraw consent for any non-clinical use of your material. Requests are made through the hospital’s patient services, and the contact page explains how to reach them.
